In order to deal with your request, the ombudswoman examine each file for the following points:

  1. Surname, first name, address, phone number or E-mail of the complainant
  2. Accurate formulation of the request
  3. Description of the decision or an action of the administrative authority against which you complaint
  4. Description of the steps that have been taken so far
  5. Were the existing opposition proceedings exhausted?
  6. The complaint relates to facts which are no more than 1 year before the complaint was lodged.
